Community Support Workers are a key part of services to persons with severe and chronic mental illness in the community. They provide support and monitoring to persons in community settings, helping to build and maintain their independence, and link to needed services. They also have a strong advocate role for the persons they serve.
ACT (ASSERTIVE COMMUNITY TREATMENT)
ACT is a team-based approach to providing intensive mental health services in the community to the most severely mentally ill individuals in Geauga County. The team works to help individuals maintain their independence in the community and decrease need for psychiatric hospitalization.
SLS (SUPPORTIVE LIVING SERVICES)
The SLS program assists persons with serve and chronic mental illness to learn necessary skills in order to live independently. This often includes, cooking, cleaning, personal hygiene, budgeting, shopping and other daily living skills. Staff provide training in the community and home environment which builds and reinforces skills necessary to remain as independent as possible. Staff do not "do for", but use role modeling, reinforcement and verbal cues to help build daily living skills, while supporting appropriate coping mechanisms and social skills.
